Case Manager – Aging & Family Support

About the role

The Case Manager – Aging & Family Support provides coordinated assistance to older adults and their families. Working part‑time in a hybrid setting, the professional conducts assessments, creates individualized care plans, and ensures that services remain appropriate and effective.

Key responsibilities

• Perform comprehensive needs assessments for seniors and their families.
• Develop and maintain individualized care plans.
• Connect clients with community, health, and social resources.
• Monitor service delivery and adjust plans as needed.
• Maintain accurate case documentation and records.
• Collaborate with healthcare providers, social workers, and other multidisciplinary teams.
• Provide guidance to families on navigating care options and support systems.

Required profile

• Experience in case management, social work, gerontology, or related fields.
• Strong client‑assessment and care‑planning abilities.
• Excellent interpersonal, communication, and active‑listening skills.
• Ability to manage multiple cases and maintain detailed records.
• Knowledge of local health, social services, and community resources for older adults.
• Bachelor’s degree in Social Work, Psychology, Nursing, Gerontology, or a related discipline; certifications are a plus.
• Fluency in Hebrew; English proficiency is an asset.
